Acute Care at Home

The Acute Care at Home Service provides advanced nursing care to support patients to remain in their own homes. The team is made up of highly qualified, experienced nurses who provide advanced assessment skills and treatment. The aim is to prevent an admission to hospital or support an early discharge.

Inclusion criteria

The patient must:

  • be over the age of 18
  • have access to a telephone at home
  • agree to treatment being given at home

The team can manage a range of conditions. These include but are not limited to:

  • chest infections
  • bronchiectasis
  • chronic obstructive pulmonary disease (COPD )
  • cellulitis
  • osteomyelitis
  • heart failure
  • endocarditis
  • pneumonia
  • tonsillitis
  • urinary tract infection or pyelonephritis
  • post-operative infections
  • discitis
  • liver abscess
  • meningitis
  • hyperemesis

Intensive nursing support

The team is skilled in providing intensive nursing support at home. Skills include:

  • intravenous (IV) therapy, including re-hydration, antibiotics, anti-emetics and diuretics
  • mid-line insertion 
  • respiratory and cardiovascular assessment and examination
  • cannulation
  • phlebotomy
  • nebuliser therapy (short-term use)
  • blood transfusions and platelets (for housebound patients only)

Referrals

Any health professional can make a referral to the team. Patients cannot self-refer.

Information for referrers

The Acute Care at Home Team is available from 8am to 10pm, 7 days a week.

A nurse co-ordinator is in the office Monday to Friday, 8am to 8pm. Outside of these hours or if there is no answer, you can leave a message. At evenings and weekends, you can also contact the team via Bodmin switchboard. Call 01208 251 300.

We hold weekly ward round meetings where we discuss each of our patients with a consultant, a microbiologist, and a specialist pharmacist.
