Diabetes specialist nurses work in hospitals and community clinics to help people with diabetes to safely manage their condition.
Diabetes specialist nurses work with Royal Cornwall Hospitals NHS Trust to identify people with diabetes patients in hospital. The nurses provide expert nursing care to patients with diabetes in hospital to support their early discharge and reduce readmissions. They also educate patients and other healthcare professionals on the management of diabetes in hospital. Diabetes specialist nurses work with GPs and other healthcare professionals.
The nurses are available Monday to Friday at the Royal Cornwall Hospital in Truro, West Cornwall Hospital, and St Michael’s Hospital. Cover is provided over 4-day bank holiday weekends.
Through our network of community hospitals the diabetes specialist nurses support adults with type1 and type 2 diabetes. The nurses will work with patients to promote a healthy lifestyle and reduce the complications associated with the condition. The nurses can support you with carbohydrate counting, pre-conceptual advice, ante natal care, and insulin pump therapy.
The diabetes specialist nurses who work in the community are available Monday to Friday between 9am and 5pm.
