Home Treatment Team

We are a service for people over 18 years old, who are experiencing acute mental health difficulties, which without intensive help may lead to a further deterioration and possible hospital admission.

We aim to help people manage and resolve their difficulties through assessment and treatment in their home, as a safe alternative to hospital admission.

We also provide support for early discharge from hospital, enabling people to continue their recovery at home.

What we offer

If you are taken onto our caseload, you will have access to support 24/7.

Following your assessment, we will discuss how best to support you and create a care plan of visits and contacts. Our input will be short term.

We aim to be flexible, accessible and sensitive to your needs, working together to aid recovery.

We work a shift pattern that may mean you are seen by various members of the team. We will attempt to minimise this where possible but we cannot guarantee that the same clinician will be available.

All contacts are recorded on your electronic records and staff visiting will have access to these prior to contacting you, so you will not need to repeat information previously given.

What can you expect?

  • 24/7 telephone access to Home Treatment Team.
  • A comprehensive assessment of your needs.
  • Short term input until the crisis has resolved.
  • Referral to other services, if needed.
  • To be involved in decisions about your care.
  • Involvement of and support to family and/or carers where appropriate.

The team is available from 8am to 8.30pm with night cover from 8pm to 8.30am.

Family and carers

Part of our involvement is to support the family, carers and networks surrounding service users. At times, services users may place limits on how much information regarding their care we are able to discuss. This does not prevent the team from listening to views and concerns of family, carers and people in the network of the service user.

If you provide regular and substantial care to someone who is using our service you may be entitled to a carers assessment.

To request a carers assessment, speak to your care co-ordinator or a member of our team.

Admission alternatives

An admission alternative can be used in 2 different ways:

  1. As an alternative to being admitted to an acute psychiatric ward.
  2. A step down from the ward environment.

The aim is to provide a nurturing environment with a home from home feel. Stays are usually for a period of 2 or 3 weeks, up to a maximum of 4.

This may be offered to you if you are experiencing a mental health crisis, and you would benefit from a short stay within a supportive environment. The focus will be on what is meaningful for you, your recovery and promoting your independence.

In Cornwall, there are currently 2 options, once a referral is received. The Admission Alternative Team will review your needs and decide which option is best placed to meet your needs.

Self referrals

Referrals to our team are through a person's named worker. Outside of office hours, a referral can be made by a GP or profession to the Professionals Line.

Carers, family members or self-referrals to our team can be made via NHS 111. Call NHS 111 and select option 2 for mental health support. People who are not currently using mental health services can also access support through NHS 111.
