Tele-health – Home Monitoring

Tele-health is an easy-to-use, home monitoring system, individually tailored to support patients living with long term conditions.

What is Tele-health Home Monitoring?

Once installed in the patient’s home, Tele-health equipment called Alfi keeps a regular check on vital signs such as blood pressure, blood sugar levels, oxygen levels, and weight, and sets up an ‘early warning system’ to detect changes.

Alfi can help people with a range of long-term conditions, including:

  • heart (cardiovascular) disease
  • lung disease eg chronic obstructive pulmonary disease
  • diabetes
  • high blood pressure (hypertension)
  • Urinary tract infections

Tele-health home monitoring helps people to reduce the risk of falls, stroke and heart failure.

Once it has been identified that you may benefit from Tele-health Home Monitoring, we will visit you to set up the equipment and how to use it. At set times of the day, Alfi will remind you to take your vital signs (blood pressure, blood glucose and the oxygen in your blood). Depending on your condition, we may set Alfi up to ask you some key questions.

Once you’ve taken your readings, this information is sent automatically via your phone line to Alfi’s Telehealth Nurse Team. A ‘traffic light’ system helps them spot any changes in your condition that might need attention – and also helps you to monitor and understand your own condition.

If your readings do show a problem, one of our team will get in touch with you to make sure you’re OK and advise you on what to do.
