Complex Care and Dementia Psychiatric Liaison
The Complex Care and Dementia Psychiatric Liaison Team are a specialist mental health service based at both the Royal Cornwall and West Cornwall Hospitals. We are a team of doctors and specialist mental health nurses. The Liaison Service forms an essential part of the complex care and dementia pathway from early diagnosis of dementia to end of life care. We also provide support to those living with a functional mental illness over the age of 75 years old.
A large proportion of our work involves providing advice to hospital staff such presentation as delirium and behavioural and psychological symptoms of dementia. We also advice on the use of the Mental Health Act, the Mental Capacity Act including Deprivation of Liberty safeguards.
In 2015 we introduced an innovative new role of CCD discharge liaison nurse. This role sits between the CCD psychiatric liaison team and the Onward care team, using specialist knowledge, to support people living with dementia to increase patient flow away from the acute hospital, to ensure patient centred care. We also provide the hospital nursing staff with advice on the use of the Mental Health Act and the Mental Capacity Act and help to plan discharge from hospital
Countywide Dementia liaison
The Countywide Dementia Liaison service provide specialist in-reach support to dementia registered care homes and most community hospitals. We focus our support toward people living with dementia.
The service provides a balance of anticipatory and proactive care whilst remaining flexible to be able to respond to emergencies where possible. The focus of the service is to support the transfer of specialist skills to generalist staff in care homes and community hospitals.
The team provide support, advice and education to staff, carers and other professionals in these settings. This enables the staff to effectively treat and care for you where you are and can help avoid unnecessary admissions into an acute hospital.
Hours of Working
Monday – Friday, 9.00 – 17.00.
Teams do not work on Bank Holidays.
