Paediatric Discharge Liaison

The Paediatric Discharge Liaison service liaise with wards, multidisciplinary teams, tertiary services and the local community services, to aid discharge of children, particularly those who have complex and technology dependent health care needs.

The Paediatric Discharge Liaison Service ensure that the child and family are at the centre of care and treatment by promoting good communication to ensure that the child/young person (where appropriate) and their family are involved in planning their care and also planning the future service delivery.

The service is based both at Truro Health Park and The Royal Cornwall Hospital, attending the wards and Emergency Department at the hospital on a minimum of 3 days each week.

The service recognises the importance of safeguarding children & young people, and is committed to communicating with community and acute health providers, and with other providers, such as Social Care, to safeguard children and young people.
The service works in partnership with the Named and Designated Safeguarding nurses and Safeguarding Doctors to ensure that good practice is maintained.

Discharge Liaison cover is provided 7.5hrs per day, Mon – Fri (excluding bank holidays), working flexibly with the Children’s Unit RCHT, Children’s Services in CFT, and, where and when appropriate, children, young people and their families. The service can be accessed via the Health Park on 01872 221444. In the Emergency Department there is a folder containing information about the service, contact details, and a book in which to leave messages to request follow up by the service regarding communication with community service providers.

The service is also accessible by approaching the Paediatric Discharge Liaison Coordinator in person.
