
The Home Treatment Teams provide a high level of support to people over the age of 18 in mental health crisis or relapse in their own home.
Home Treatment Teams offer an intensive package of support to people on their caseload who are experiencing acute problems with their mental or emotional health. They will assess the person at home and decide if they need a hospital admission or whether they can be treated at home to avoid admission where it is possible and safe to do so. Intensive support is offered over an agreed period of time, this can include; practical support, help with solving problems, medication, relapse prevention and crisis planning. The Home Treatment Team works between 8am and 10.30pm with night cover from 10.30pm to 8am.
The team are available 24 hours a day to provide Mental Health Act Assessments.
Home Treatment Teams provide support based each person’s individual needs. They work closely with Community Mental Health Teams and Care Coordinators. We accept self referrals or you may be referred by your CPN, GP, or other health services. We have two teams which support people in the east and west of the county.
For self referrals in office hours, in the first instance please contact your local Integrated Community Mental Health Team (iCMHT). Outside of office hours please contact Home Treatment Team.

Support Matters Cornwall is an out-of-hours helpline operating from 5pm to 9am on weekdays and 24 hours a day on weekends and bank holidays. The service is open to all people aged 16+ under the care of the Trust’s mental health services only. Call free on 0800 001 4330.
